Free transportation to medical appointments

WEBMassHealth members are eligible for free transportation to and from medical appointments when public transportation isn't an option. To request a ride, when scheduling the appointment, ask your doctor's office to submit at Prescription for Transportation (PT-1) form to the MassHealth Customer Service Center on your behalf.
