As the question asked for the 4 general principles of prevention, I think am I correct in assuming that they are talking about Elimination, Subsitution, Eng Controls and Admin control. The Introduction to H&S by Phil Hughes on page 81 gives 11 elements to the hierarchy of control.

Hazardous malfunction

'Ask.com' defines it: A hazardous malfunction is usually the result of a breakdown of the hazardous control systems in a machine or equipment. It is caused by dangerous occurrences in the machine or equipment hence forcing the control systems to break down.
Medical Treatment Case (MTC) or First Aid Case (FAC)
